Thanks to an incredible $1.5 million gift, the Slaight Family Foundation Recovery and Reintegration Program is helping to close the gap in care that young people who have experienced psychosis often face after being in the hospital—meeting a critical need for our west-end community.
“This unique and comprehensive program serves patients between 18-29 years old who have experienced psychosis and are in the process of recovery and reintegrating into their community. An important focus of the gift is strengthening connections between the hospital and local mental health organizations to promote the continuum of care post-discharge. St. Joe’s and LAMP Community Health Centre are long-time collaborators, having worked together on other programs, and they are an exceptional partner for this initiative, housing the program in their space in the west and helping connect these vulnerable young people to programming and services that provide support.
